I didn't get it either. But when you google on 'Flat Fatima' it appears to 
be a big picture you can buy.
They advertie it as something you can put into a news cast or other media 
article for dramatic effect.
(Assuming that you want a picture of a picture of a distressed older woman, 
presumably a muslim.)
